Privacy

Privacy policy.

Last updated 7 October 2026. This covers the website. Information you give us as a patient is covered separately by our HIPAA Notice of Privacy Practices, which you receive at your first visit.

What the website collects

Two kinds of information.

What you send us. If you complete the appointment-request form or use the chat, we receive what you type — typically your name, a phone number or email address, and a sentence about what you are coming in for. We ask you not to send clinical detail, insurance member IDs, Social Security numbers or card numbers through this site; none of it is needed to arrange a call back.

What is recorded automatically. Standard web analytics: pages viewed, approximate location from your IP address, device and browser type, and how you arrived. This is collected through Google Analytics 4 and is not used to identify you by name.

How it is used

What we do with it

  • To contact you about the appointment or question you submitted — that is the only reason the form exists.
  • To verify your insurance eligibility before a visit, where you asked us to.
  • To understand which pages people find useful, in aggregate, so the site answers more of the questions patients actually have.
  • We do not sell your information, and we do not share it with advertisers or data brokers.
  • We do not use what you send us to build an advertising profile of you.

Who else sees it

Only the service providers that make the site work, and only to the extent they need to: our hosting provider, our email delivery provider, the analytics provider named above, and the provider behind the chat assistant. Each processes data on our instruction.

We disclose information beyond that only where the law requires it.

Form submissions and chat transcripts are kept for two years and then deleted, unless you become a patient — at which point the clinical record is governed by Florida medical-records law and our HIPAA notice, not by this policy.

Your choices

What you can ask us to do

  • Ask what we hold about you, and ask us to correct or delete it. Write to [email protected] or call (305) 555-0180.
  • Opt out of analytics entirely. We honour the Global Privacy Control signal: if your browser sends it, no analytics tag loads at all.
  • Decline the chat. It is optional and nothing on the site requires it.
  • Florida residents: the Florida Digital Bill of Rights gives you rights of access, correction, deletion and opt-out. We apply those rights to everyone who asks, wherever you live.

Children, security and changes

This website is not directed at children under 13 and we do not knowingly collect their information through it.

The site is served over HTTPS and submissions are encrypted in transit. No website can promise perfect security, and we will not pretend otherwise.

If this policy changes we update the date at the top. Material changes will be flagged on the page rather than made quietly.

Questions: [email protected], or (305) 555-0180 during office hours.
