Abnormal Pap and HPV
An abnormal Pap result.
An abnormal Pap is not a cancer diagnosis, and a positive HPV test is not a disease. Both are reasons for a specific next step, and this page says which step follows which result.
How this is approached here
What follows which result
-
01
Management follows ASCCP risk-based guidance
The next step depends on this result together with your history, not on the cytology label alone.
-
02
HPV-negative ASC-US is usually a return
To routine screening, not a colposcopy.
-
03
Persistent high-risk HPV
Or HPV 16 or 18, moves to colposcopy — performed in the office, taking about fifteen minutes.
-
04
CIN 1 is followed rather than treated in most cases
Because most of it regresses without intervention.
-
05
CIN 2 and CIN 3 are treated
Usually with LEEP in the office. Fertility and pregnancy plans change that conversation, so they are part of it.

Results are explained by a clinician

Screening intervals
How often this should happen
Screening starts at 21 earlier testing finds HPV that would have cleared on its own.
21 to 29 cytology every three years, or primary HPV testing where it is available.
30 to 65 co-testing every five years, or cytology alone every three.
Over 65 with an adequate negative screening history
Screening stops. That is a guideline, not a cost-saving.
After a hysterectomy for benign disease
With no history of CIN 2 or worse: no further screening needed.
Immunosuppression
HIV and in-utero DES exposure all mean a different, more frequent schedule.
